Deposit Form

We invite everyone who visits Photoville to deposit a photograph.
For those of you who are unable to attend the event, here is your chance to be part of the Depository.

INSTRUCTIONS:

1. Download the form (PDF)
2. Attach a photograph and fill out the form
3. Return the form for processing. You may either:
   A. Scan the form and email it to [email protected]

   B. Place it in an envelope and mail it to 111 Front Street, Suite 204 – Brooklyn, NY 11201 – USA

4. Once the form has been processed, you will receive a confirmation “stamp” email(usually within 48 hours)
